Makoon Transition Inc. is here to walk alongside you as you work toward healing, independence, and reunification with your children. Applying to Makoon is simple, and we’re here to support you every step of the way.

Who Can Apply

Makoon is open to:

● Parents involved with the child welfare system who are working toward reunification with their children

● Families needing a safe, supportive space to heal and build stability

● Parents seeking cultural, emotional, and practical support during a time of transition

If you’re unsure whether Makoon is the right fit for you, reach out, and we’ll help you explore your options.

How to Apply

1. Referral through Child and Family Services (CFS):

● Your CFS worker can complete a referral on your behalf using our

● Makoon staff will connect with you and your worker to begin the intake

2. Self-Referral:

● Parents can apply directly by contacting our Intake Team or by filling out the Intake form below.

● We’ll walk you through the application process, answer your questions, and provide support.

What to Expect After You Apply

The process is designed to be supportive, not overwhelming. Here’s what will happen:

● Step 1: Submit your completed Intake form (online or by print).

● Step 2: Intake staff will contact you to gather details and explain what Makoon

● Step 3: We’ll arrange a meeting with you (and your worker, if applicable) to discuss your needs and goals.

● Step 4: If Makoon is the right fit, we’ll guide you through the move-in process and help you get settled.
