Position Summary:

Under the supervision of the Client Intake Supervisor and Director you will be responsible for Makoon Transition Inc. Client Intake processes and management of intake applications, responding to all inquiries in a timely manner.

Responsibilities:

  • ● Management of Makoon Transition Inc. Client Intake email, consisting of responding daily, screening applications, and communicating with the Director on possible applications for our program.
  • ● Client Intake screening processes include knowledge of Makoon Transition Inc. Mission and Vision, programming available for clients.
  • ● Communicate daily with Director and management on client intake list, waitlist, move in dates and updating of all digital files.
  • ● Coordinate move in of new clients with Director, Program Manager, Case Manager Supervisor and Family Support Worker Supervisor.
  • ● Work closely with Child and Family Service agencies regarding all intake processes.
  • ● Expert communication and knowledge of the child welfare system and social services.
  • ● Do Drug testing of all possible clients before acceptance into the Makoon Transition Inc.
  • ● Able to work within a team or independently when needed.
  • ● Creating the file with all documentation necessary for client intake.
  • ● Scheduling appointments with all collaterals involved for site visit and client intake meetings, updating calendar invites to management.
  • ● Attend and contribute to the success of strategic planning and mandatory staff meetings, team building, family BBQ and events.
  • ● MUST have your own transportation to and from work.
  • ● Ensure no unapproved visitors enter the premises.
  • ● To be prompt and ready to work everyday in a positive manner.
  • ● Sign in and out daily for accountability of attendance and tracking of time.
  • ● Must remain on site on your assigned shift.
  • ● Maintain a professional relationship with all families and other collaterals internal and external of Makoon Transition Inc. not crossing any ethical boundaries.
  • ● Follow all rules, policies and regulations enforced by Makoon Transition Inc being a positive role model.
  • ● Follow the chain of command and communications process internal and external of Makoon Transition Inc.
  • ● Must complete a daily shift summary by end of day and email to the team.
  • ● Keep emails organized and filed.
  • ● Protect the integrity and work culture of Makoon Transition Inc.
  • ● Do not contribute or start Lateral Violence.
  • ● When responding to emergency or crisis situations and/or contact 911, Program Manager and Management.
  • ● Ensure no drugs, alcohol or illegal substances enter the building.
  • ● Respond to family’s questions or concerns with empathy and professionalism.
  • ● Log in to and update Client Intake Applications on the Makoon Transition Inc. Database.
  • ● Other duties as assigned.

Qualification:

  • ● Bachelor of Social Work with 3-5 years experience in Child Welfare.
  • ● Valid Class 5 Driver’s Licence and access to a vehicle.
  • ● CPR First Aid and de escalation training.
  • ● Mental Health First Aid.
  • ● Non-Violent Crisis Intervention.
  • ● Computer Literacy, including Google Workspace, MS Word, Excel, email, and data entry.
  • ● Expert verbal, written and communication skills to articulate care plans to clients, support staff and professionals regarding child welfare and social services.
  • ● Knowledge of Google Workspace and Microsoft Office with the ability to navigate shared drives.
  • ● Strong morals and ethics, along with a commitment to staff and client privacy.
  • ● Able to follow the missions and vision statements of Makoon Transition Inc. for the success of the families.
  • ● Strong organized and time management skills are a must to work with multiple families.
  • ● Must provide criminal record, Adult and Child abuse registry checks.
  • ● Agree to random drug testing for illegal substance use and alcohol for work safety.

Must Provide:

  • ● First Nation applicants are encouraged to self-identify as being of First Nation descent.
  • ● Three references must be from previous employers.
  • ● Must clearly meet all the requirements posted in Client Intake requirements.
  • ● Must have a complete cover letter and resume.
